Marker Reset Form - New Live Oak Cemetery

 

Cemetery Preservation Group, Inc. is pleased to offer professional services to reset and level any markers in New Live Oak Cemetery that have been knocked over or displaced as a result of the January 12th 2023 tornado. We are committed to providing this valuable service to our community free of charge. However, please note that the free service does not include repairing broken markers or extensive cleaning.

To ensure the safety and accountability of our operations, our insurance company requires the completion of this form before we can proceed with any work. Some markers may have incurred scratches or chips that are not be immediately visible while they are laying face down, and there is also a possibility of cracks or internal fractures due to impact from fallen trees that may open up when the markers are moved. We want to assure you that we will exercise the utmost care during the reset process to avoid any further damage to your loved one's grave marker. Together, as a community, we can preserve the memories of those we've lost and honor their legacies.

 

By filling out the form below, you acknowledge that Cemetery Preservation Group, Inc. cannot be held responsible if any markers exhibit damage once they are moved and reset. Furthermore, you confirm that you either own the listed marker(s) and/ or have the authority to complete this form on behalf of your family. Your understanding and cooperation are greatly appreciated as we work diligently to restore the cemetery to its rightful state.

IMPORTANT: When filling out the form please give a brief description of the marker that needs resetting. Since some of these markers are face down and we are unable to see the names, please include names of adjacent or nearby markers when possible.
